Registration Information

Registration is a two-step process:

Register online via our secure website, using the username and password created for Tryout Registration:  https://www.gotsport.com/asp/application/reg/Default.asp?ProgramID=4612&Type=PLAYER 


Required forms for Boys' Teams:
IYSA EMERGENCY MEDICAL RELEASE
NISL PLAYER REGISTRATION FORM
US CLUB REGISTRATION AND MEDICAL FORM

Required forms for Girls' Teams:
IYSA EMERGENCY MEDICAL RELEASE
IWSL 2010_2011 PLAYER REGISTRATION

In addition to the required forms, we will also need:

• Copy of Medical Insurance Card (front and back) - for ALL players
• Copy of Birth Certificate - for all players NEW to Galaxy Soccer Club
• NEW Electronic photo for ALL players - please email ASAP to Galaxy Soccer Club: galaxysc@sbcglobal.net ). Photo should be a head and shoulders shot (no other people in the picture, no hats, sunglasses, etc). A scanned copy of a school photo will also work well.
• $150 towards fees (assuming $250 deposit is already paid), plus payment in full for uniform order

Have your child sized for a new Galaxy uniform. Samples are available at the Soccer Post in Naperville. Be sure to try on both jerseys, shorts, and practice tshirts. The baisc uniform is two jerseys, one pair of shorts, two practice tshirts and two pair of socks. The backpack, warmup jacket and pants are optional items.
