2011/2012 Soccer Year Registration Information

Registration is a two-step process - both steps are required for all players:

Register online via our secure website, using the username and password created for Tryout Registration:  https://www.gotsport.com/asp/application/reg/?ProgramID=8702&Type=PLAYER . 

Required forms for Boys' Teams:
IYSA EMERGENCY MEDICAL RELEASE
NISL PLAYER REGISTRATION FORM
US CLUB REGISTRATION AND MEDICAL FORM

Required forms for Girls' Teams:
IYSA EMERGENCY MEDICAL RELEASE
IWSL PLAYER REGISTRATION

In addition to the required forms, we will also need:

• Copy of Medical Insurance Card (front and back) - for ALL players
• Copy of Birth Certificate - for all players NEW to Galaxy Soccer Club
• Electronic photo for all NEW players - please email ASAP to Galaxy Soccer Club: galaxysc@sbcglobal.net ). Photo should be a head and shoulders shot (no other people in the picture, no hats, sunglasses, etc). A scanned copy of a school photo will also work well.
• HS Boys need to complete registration online and on paper, and send in $150 towards fees (assuming the $250 deposit is paid), by October 19th.
