• Team Manager Checklist
• IYSA Tournament Roster (required for ALL teams)
• IYSA Guest Player Form
• IYSA Travel Permit
• IYSA Emergency Medical Release (required for ALL teams)
• Form NT (required for all US Club teams)

The above forms must be completed, signed as needed, and mailed with other required items listed on the Team Manager Checklist to the Galaxy Soccer Club by Friday, September 2nd.  Please mail to:
